2. SCTEX up to the Tarlac city interchange (La Paz). Toll P93. Turn left to the Santa Rosa-Tarlac Road; after about 9km, slight right to Matatalaib Road. Then merge to MacArthur Hway. This way, you get to bypass the city proper. (map)
3. Fuel. Gas was pricier on the NLEX, but much cheaper on the MacArthur Hway (aka Manila North Road). My best find was at the Shell station in Udiao, Rosario, Pangasinan: unleaded P54.91 -- P57.75 in Manila and almost P59 on NLEX. (The map only shows Petron; Shell is across the road.)
